"The Community Asthma Prevention Program currently serves the West, Southwest, and North Philadelphia areas of the city. We may be able to offer home visits to your family if your child is between 2 and 16 years of age and has had a diagnosis of asthma. Your child also needs to have persistent asthma and be on controller medicine."
